Humour

I came across this really stupid site that my friend recommended I read... And having nothing to do, I did!

Carotid Endarterectomy

Ah. My first educational post.

It'll detail the surgery that I observed the other day, a carotid endarterectomy. Sounds really complicated? Well, sort of.

Like many surgeries, simple errors can have tremendous consequences - particularly because the body is packed more tightly than sardines. Nerves, vessels, lymphatics, muscles, etc. One slip of the scalpel and that's it.

In actual fact though, the term just means cutting open the carotid artery (Internal) in the neck; scraping out all the plaque that has formed; then stitching it all back up. Yeah, Yeah. I know you're all saying "Chey", but that procedure took 2 hours.

Anyway, we are operating on the carotid triangle on the anterior cervical region.

Hehe. Let you take a look at this little teaser picture first. That's me in the middle. How? Look like a surgeon or not?



Here you can see the constriction in the Internal Carotid Artery. Now as I have noted earlier, this surgery is actually pretty risky and you would not elect to have it most of the time. However, the patient had suffered a stroke and this is being done to prevent additional incidences of that.


Nothing much here. This picture just shows the initial cut. Note that all the equipment you see here are sterile - in other words, free of bacteria and viruses.


In this picture, you can observe the anatomy of the carotid artery. It's the one that they always like to show in movies - the evil guy severs it and blood spews out like crazy. In actual fact, it's not as dramatic.

The surgeon has tied down the arteries so that they don't move about too much.

The first horizontal red line above the carotid arteries is the Hypoglossal Nerve (CN XII).

A more detailed picture. Note the Vagus Nerve (CN X). That one controls nearly all of the parasympathetic nervous system in the body - including your heart. Cutting or damaging it is NOT recommended.

The cartoid arteries, the internal jugular vein, and the Vagus Nerve (CN X) also make up what is known as the carotid sheath.


The atheroma is what you get in return for too much Cha Kway Teow, KFC, or Kway Chap with lotsa fatty pork. You can see how the presence of it makes the diameter of the blood vessel smaller - hence allowing less blood through.

Just below the middle blue tie is where the atheroma has been partially dissociated from the artery wall. Gives you an idea of its thickness eh?

That's it when removed from the artery.


Here we have the artery all stitched up and we're now ready to close off the skin.

Anyway, that's all for the surgery that I attended! I actually went to observe a kidney transplant too, but we forgot to bring a camera for that.

Hope it has been interesting! Note that I haven't studied Head & Neck anatomy yet, so there might be mistakes. Don't take this to be the Bible!

Spring Redux

Wow.

It's been really warm nowadays. Hasn't dropped below 15 Deg C the past two days - even at night.

Looks like spring is finally here to stay!

A long spring drizzle should serve as a warm welcome to the new season...

On a side note, I attended a lecture for Group B only student today. The rumours are indeed true! Group B has all the hot babes. Screw Group A.

Anyway, I have something to share with everyone...

Take a look at an apple tree.

There might be five hundred apples on the tree, and each apple has about ten seeds. That's a lot of seeds!

We might ask, "Why would you need so many seeds to grow just a few more apple trees?"

Nature has something to teach us here.

It's telling us: "Not all seeds grow. In life, most seeds never grow. So if you really want to make something happen, you had better try more than once."

This might mean that:

  • You'll attend twenty interviews to get one job.
  • You'll interview forty people to find one good employee.
  • And you might meet a hundred acquaintances just to find one special person.
When we understand the "Law of the Seed", we don't get so disappointed.

We stop feeling like victims. We learn how to deal with things that happen to us. We just need to understand them - and work with them.

I guess this means that I did pretty well overall. Have to admit that I was pretty worried because for this test, I didn't have the urge to study much. Only did 9 hours of studying.

Anyway, it's all well and good. Hopefully, I do as well or even better in the final exams for Year 1.

In other news, I got to eat a cake again today!

Haha. I was just passively mentioning to her that I like her cakes on Monday and viola! She made one today. Must admit I felt kinda bad when the class asked why she brought a cake today and she replied it's because of what I said. Too patronising perhaps.

Rosewater Cake. Mmm... She says it's a love cake or something that Persians used to make. Anyway, it tastes really nice - I think she can open a bakery if being a doctor becomes boring. Just a pleasant surprise on an otherwise routine Friday.

Animals 'R' Us

I had a tutorial today on Ethics titled "Animals, Personhood & Ethics".

Pretty interesting and all - philosophical; it was a getaway from the usual Science stuff.

We first talked about a scenario where a couple has a newborn who was blind, deaf and had brain damage.

What choices do they have?

Quality of life or Sanctity of life?

Expectedly, most people believed that the quality of life is probably more important than the latter.

What's interesting is that this has not been the case throughout history. Since the earliest Western civilisations, the idea was always that life was sacred - to be preserved and prolonged at any cost. This was probably heavily influenced by the Christian missionaries then, who preached that life is a gift that must be cherished and treasured; I guess the people then just took it way too literally.

The paradigm shift only came about in the past century, when medical science evolved to a state where a person, despite being brain dead, could be kept alive indefinitely. It raised the questions of quality of life, euthanasia and suffering - none of which could be resolved by the old ideas.

Of course, those are all subject to personal opinion; and while I have not decided if I would choose to die over suffering (I hope I never have to be made to suffer in my life), I do know that I will never assist someone in killing himself, regardless of how much I believe it would alleviate their pain.

But, that is not the disturbing thing. What shocked me was that humans are so "species-ist", to say the least.

Personhood. Or what makes one a person.

  • Ability to reason
  • Free will (To make choices)
  • Sentience (Ability to feel emotions and pain)
  • Consciousness
  • Memories
  • Self Awareness
Those were just a few of the points listed by my tute mates as what makes a human (A sort of animal) a person.

But who is to say that the list doesn't apply to an animal?

Let's take an elephant for example. They can definitely do all of the above.

Yet what makes us, in all our hubris, believe that we are so different from "animals"?

The answer is choice.

Because we choose to. It's the same reason why Lassie isn't just a dog but a friend; or why Garfield is more than a fat, orange cat. It is because we choose to associate meaning, that we choose to classify things, that we choose to see differences when in fact, were the differences to be considered in context, they would be non-existent.

It is sad when your tute mates tell you the tutorial is boring not because it had no medical relevance, but because there is no point debating the issue in the first place since we are superior.

Whoever said that? Who taught them that? Society? Religion?

Is the lack of technology, science and metaphysics (Read: Religion and Beliefs) a sign of evolutionary inferiority?

I don't know about everyone. But I can put myself in an monkey's shoes and think.

I have a social structure made up of young, females and alpha males. I can recognise myself in a mirror. I know how to use tools. I can feel pain and express emotion on my face. I can choose to do what I want to. I can remember what's past.

Are we really all that different from "animals"?

My Big Fat Aussie Post!

Heya! This is so overdue and I'm terribly sorry! However, I've been busy (Lazy) lately and have had no time (Lack of motivation) to post an update. But... But But But, I'm all psyched up now and this will be my LARGEST post yet!

Sadly, not much pictures. Haha. As you all know I'm camera shy because I'm afraid my good looks will sway people's world view, so no choice but to hide myself.

Anyway, first up is my Rural Placement to Mildura! (Which actually happened 1 freaking month ago!)

Ok for the uninitiated, Australia has more than 640 km² of land; and thus, is divided into Rural and Metropolitian areas!

Mildura is a nice, sweet town that straddles the states of Victoria, New South Wales, and South Australia! Pretty confusing I'd admit.

It's pretty quiet and isolated from the big cities, and thus, is a totally different living experience all together. The city centre is a 5 min walk from your home, there is only ONE pub in the whole freaking place, and everyone is a farmer who grows grapes (Ok maybe not everyone).

Regardless, it has it's charms, and I really like that place!

The days were mostly spent going to hospitals (Where I can't take pictures), so I shall move on to the recreation days!

First up is Orange World, which was basically a big, fat orange farm growing well, oranges.


That's me in case you have no idea...

Yeah anyway, we just had lotsa oranges and stuff. And after that, we went on this cutey river cruise!

I'd bet that some of you have never ever seen a canal lock before! There you go! It's actually pretty dull and took quite a while. Oh well, at least I can claim to have tried that.

So that about sums up my Mildura trip. Other than this incident in Orange World where I ended up being called "me" and the name stuck with this Ang Moh girl. It's been a month and she still doesn't stop it! Mmm... I'll get her for it someday.

Right. Besides that I've been studying hard for my exams! Did you know that there are 19 muscles in your forearm (That's the part below the elbow) alone?! And I have to memorise all of them! I can't stop gushing about Medicine and how happy I am to be in this course.

Oh yeah, we had a Hall "Mock Wedding" function last Wednesday. Quite fun really, we all dressed up and went for dinner and crap.

This is me then (Okie obviously taken in my room cos it was too dark outside)

The expression was on purpose of course.

In other news, it's now Spring! Finally, the days don't turn dark at 5 pm in the evening (Bitch!) and we don't have to look like roosting pigeons who can't fly.

Mmm... I also got to observe a kidney transplant! Live donor. How cool is that? Haha. You know those surgeons on TV wearing the wear masks and gown? I looked like that - now I know the secret to why doctors in those photos look so good! Because it hides everything! Of course, it's not like I have anything to hide.

Right. That's all I can manage for now. I'll post more photos once my friend sends them to me. In the meantime, stay sharp and happy! I'm not sure when I'll update my blog again. Hehe.
